The Vice President of Aftercare & Coordinated Care is a key position responsible for comprehensiveleadership, management and performance improvement of the organizations After Care & Coordinated Care Center. This position will work closely with Medical Leadership, Population Health &Value-Based Contracting, Quality Improvement, and Clinical Operations to identify opportunities to improve patient outreach, engagement, activation and coordination of care in support of population health management goals and clinical support of site operations.?The VP will improve the Center?s operating efficiencies and financial performance while driving high-quality patient care and outcomes 3 main areas they are responsible for: 1 - run the call center 2 - care coordination & support 3 - Care management/navigation ? The primary duties and responsibilities are: ? Driving the Mission and Vision of the organization to serve kindness. Ensuring outstanding patientexperience at all touch points within Aftercare, Population Health and Coordination of Care ? Ensure management of Aftercare & Coordinated Care provides an optimal environment for providers to impart high quality, engaging and convenient patient care ? Establish standards of performance for the operation and management of the Aftercare & Coordinated Care Department to achieve the stated performance standards ? Develop, Implement and audit policies and procedures related to flow of information and systems used within the Aftercare & Coordinated Care Department ? Improve the Aftercare & Coordinated Care operating efficiencies and financial performance while driving high-quality patient care and outcomes ? In partnership with SVP of Operations, manage strategic planning process to ensure service requirements and levels are met across the organization ? Responsible for the preparation and performance of financial budgets in addition to monthly financial projections and analysis for the Aftercare & Coordinated Care Department ? Drive financial metrics, accountability for bottom line results, and consistent application of cost control performance within the Aftercare & Coordinated Care Department and its various arms ? Develop and monitor a key performance indicator (KPI) dashboard including metrics and measurements for the department. ? Strategically approach and challenge each aspect of the operational process to improve margins ? Develop best practices for operational metric measurement/achievement ? Provide a work environment that promotes positive energy, creativity and teamwork ? Ensure the Aftercare & Coordinated Care Department is up to date on all Billing, DOH, Dept. of Labor, and other legal compliance requirements ? Maintaining patient confidentiality in accordance to policy and procedure as well as HIPAA requirements ? Be a role model of courtesy and respectfulness when interacting with team members, patients, and family members ? Partner with corporate departments (e.g., Site Operations, IT, Payroll, etc.) for support and escalation of operational, staff, and/or patient issues ? Analyzing and reporting on various business metrics ? Maintaining a working knowledge of clinical, technical, and business knowledge in the urgent careand specialty industries ? ? Qualifications ? ? 7+ years of management experience a must (urgent care, medical practice, hospital, retail, hospitality, supply chain, business administration or finance) ? Experience managing a call center operation is a plus ? Bachelor?s Degree in Hospitality Management, Public Health, Hotel/Restaurant Management, Business or other applicable degree preferred (will consider relevant management experience in lieu of degree) ? Able to drive improvements in operational processes ? Must exhibit strong data analysis capabilities ? Experience in Multi-Unit Facility setting is a plus ? Has an entrepreneurial spirit and takes a proactive approach to business operations ? Must exhibit passion for outstanding results and compassion for those we work with and serve ? Exceptional patient/customer service skills ? Happy and positive thinking, able to project this attitude around others ? Ability to work in a fast-paced/ever-changing environment ? Strong computer skills (PC, Microsoft Office programs)
